Enroll in Standard Option; Earn Wellness Rewards

All new members are enrolled in HealthShare's Standard Option. 

HealthShare members must be committed to being an active participant in their own health care.  The goal of HealthShare is to provide affordable, appropriate health care to improve each member’s long-term health.  HealthShare encourages preventative care and encourages tests that are recommended for persons of similar age to prevent potentially serious medical problems before they occur.

If a member currently has, or is diagnosed with a persistent or chronic condition, the member is required to meet with HealthShare’s Nurse Care Manager at least once per coverage year.  The member must also agree to comply with a treatment plan and work with physicians in the appropriate course of treatment.  If a member does not follow the recommended protocols, treatments or recommendations, or make lifestyle changes that will improve health, the services received may not be covered and the member would be financially responsible for them.

 

Wellness Rewards Option

The Wellness Rewards Option is designed to help you take careful, managed steps toward improving your long-term health.  You will also save money along the way, as Wellness Option participants pay a lower monthly premium and have lower co-payments for the services they receive.  The Wellness Option requires a commitment on your part, but you are not alone.  Your Care Manager and HealthShare staff will provide you with the help and encouragement for you to be successful.

For HealthShare members willing to actively work on improving their health by setting health/wellness goals for the year and participating in classes and health related activities, HealthShare will reward those members with reduced monthly rates and copay amounts. The Wellness Rewards Option is $113 per month for adults and $84 for children 18 and younger. For the employee, the business contributes $60 toward the employee’s $113 monthly cost, leaving $53 per month as an employee pre-tax payroll deduction. 

Below are the tasks members need to initiate in order to earn the Wellness Rewards Option:

  1. Call and schedule a 1/2 hour meeting with HealthShare's Nurse Care Manager within 1 month of the member's initial HealthShare enrollment date. To schedule this meeting call (218) 336-5706.

During this meeting the member and Nurse Care Manager will:

  • Review the completed Health Risk Appraisal that was completed during enrollment. Identify any chronic or potential health conditions and discuss options to address these.

  • Review any current prescribed medications and make sure they are covered by HealthShare, and if not, evaluate other options suchs as Pharmacy Assistance programs, generic or similar drugs that are listed on the HealthShare drug formulary.

  • Set two health / wellness goals to work on during the current coverage year.

  • Answer any health concerns or program questions the member may have.

  1. Within 12 weeks after the above meeting the member is required to give an update to HealthShare's Nurse Care Manager about the progress made on the goals they set for the current year.
  2. Engage in either two health related classes or two health related activities, or a combination of  a class and an activity within 10 months of the member's coverage start date and return a completed Health Education / Activity Participation Form to show this requirement is completed.

Once steps number 1 and 2 above are completed the member is eligible to change their coverage to the Wellness Rewards Option and enjoy the reduced monthly payment and co-pay amounts.

In order to stay enrolled in the Wellness Rewards Option step 3 must be completed and all 3 steps must be repeated each year upon your HealthShare renewal date.

If any of the steps are missed or not completed your coverage will revert back to the Standard Option with higher monthly costs and copays.
